1. Complete and sign/authorize the Credit Application form .  If you have any questions about the form, please contact Accounting at 650.631.7776.

2. Once Sweet Production has verified your credit history, you will be assigned a unique customer number.  If you forget your customer number in the future, please contact Customer Service at 650.631.7777.

3. Please complete the Credit Card Authorization form  to allow Sweet Production to charge your credit either once a week or once a month depending on your financial cycle.  If you need Sweet Production to charge another credit card, please complete a new Credit Card Authorization form and include your customer account number on the top.  Please fax the Credit Card Authorization form to 650.631.7770 to the attention of “Accounting Department” or email a completed form to accounting@sweetpinc.com
